What are the responsibilities and job description for the Quality Outreach Lead Coordinator position at ND Health and Human Services?

Medical Services is hiring a remote Quality Outreach Lead Coordinator that will be in North Dakota.

As our Quality Outreach Lead Coordinator, you will have the opportunity to directly influence member health outcomes by improving access to preventive care and strengthening provider engagement. You will support the State of North Dakota's vision of becoming the healthiest state in the nation by advancing quality improvement initiatives that enhance the overall health and well being of the citizens we serve.

As the Quality Outreach Lead Coordinator for the Quality Outreach team, you will play a central role in fostering a culture of data driven performance improvement focused on clinical quality, operational efficiency, and both member and provider experience. You will promote primary care delivery and preventive services through a Patient Centered Medical Home (PCMH) approach, helping ensure consistent, high quality care across the system.

You will coordinate and oversee the delivery of Early and Periodic Screening, Diagnostic and Treatment (EPSDT) services, ensuring compliance with state and federal regulations. In this role, you will serve as a key resource for the team, guiding efforts to promote preventive and primary care services to both providers and members.

You will identify performance gaps and develop strategic recommendations that improve operational effectiveness, care outcomes, and the overall patient experience. This work will require strong collaboration with internal and external partners to align efforts and drive meaningful improvements.

To succeed in this position, you will bring experience in quality improvement, care coordination, and population health initiatives. You will communicate effectively across a range of stakeholders and use strong analytical skills to interpret data and translate it into actionable strategies. Your leadership will support the team in advancing quality care and achieving measurable results.

To be considered for this position you must have the following a bachelor's degree in a related health and human services field (social work, population health or nursing).

You are required to become Patient Centered Medical Home Content Expert certified within one year of employment if you are not already.

Location

This position is remote in North Dakota, and you must have the ability to travel as needed.
