LVN / LPN Case Manager

A rewarding travel position is available for an experienced LVN/LPN Case Manager in Rochester, New York. This full-time role offers guaranteed 40 hours per week with an 8-hour day shift, Monday through Friday, from 8:00 a.m. to 4:30 p.m. The role is mobile, requiring travel to patient homes, hospitals, and provider offices, with a home base at the Riedman Administrative Campus. This position focuses on supporting the Transitions of Care program, aiming to reduce 30-day readmission rates by collaborating with a multidisciplinary care team, including RNs, Social Workers, and Pharmacists. Responsibilities include appointment scheduling, biometric data monitoring, patient education, attending care team rounds, and connecting patients with community resources. This is not a hands-on nursing role, but clinical knowledge of CHF, COPD, and complex comorbidities is preferred. The role also involves maintaining a monthly caseload of approximately 25–30 patients, frequent patient contact, and providing updates during virtual care team meetings.
