Position Summary
We are seeking experienced Licensed Practical Nurses (LPNs) for full-time positions in our long‑term care / nursing home facilities in Superior and Fond du Lac, Wisconsin. Both roles require a commitment to providing high‑quality, compassionate care to residents in a skilled nursing environment. Candidates must be available to work every other weekend (EOW) and meet the specific shift and experience requirements for their chosen location.

Shift: 8‑hour Evening and day shift
Every Other Weekend (EOW) required
Experience requirement: Minimum of 2 full years of recent experience as an LPN .

Responsibilities
- Deliver comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and other treatments following physician orders and safety protocols.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques.
- Monitor patients’ conditions closely, including acute pain management and airway management for those with respiratory needs or ventilator support.
- Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute care services.
- Maintain accurate medical documentation adhering to HIPAA regulations and ICD coding standards (ICD-9/10).
- Provide specialized care for patients with dementia or Alzheimer’s disease, including memory care strategies and behavioral health support.
- Support triage activities by assessing urgent patient needs in emergency room or urgent care settings.
- Work with individuals with developmental disabilities or those requiring hospice or palliative care services.
Skills
- Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable.
- Strong knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques.
- Proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate medical documentation.
- Skilled in venipuncture (phlebotomy), injection administration, catheter care, and specimen collection/processing.
- Ability to perform physical examinations and assessments effectively while demonstrating excellent patient care skills.
- Familiarity with infection control practices and aseptic techniques to ensure patient safety.
- Experience working with diverse populations including pediatrics (neonatal ventilator), geriatrics (memory & dementia care), and individuals with disabilities or developmental challenges.
- Knowledge of pain management strategies and acute care procedures such as spinal taps or gastric lavage when applicable.
- Capable of working independently while collaborating seamlessly within multidisciplinary teams across hospital medicine, outpatient clinics, home & community care settings.
