- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and transfer devices.
- Take and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates, ensuring timely reporting to nursing staff.
- Support medication administration under supervision, adhering strictly to protocols and maintaining detailed medical documentation within Electronic Health Record (EHR) systems such as Epic or Cerner.
- Provide specialized care for patients with complex needs including dementia, Alzheimer's disease, memory care requirements, behavioral health challenges, or those in hospice or palliative care.
- Perform catheter care, wound dressing changes, and aseptic techniques to prevent infection and promote healing.
- Assist with medical procedures such as phlebotomy (blood draws), specimen collection, and basic life support (BLS) when required.
- Monitor patient physiology and respond promptly to changes in condition within acute care environments like ICU or Level I Trauma Centers.
- Support patient mobility using assistive devices like Hoyer lifts or transfer belts while ensuring safety standards are met.
- Maintain accurate medical records and documentation of patient interactions in compliance with HIPAA regulations.
- Collaborate effectively with multidisciplinary teams across inpatient, outpatient, emergency medicine, pediatrics, geriatrics, and behavioral health units to ensure comprehensive patient service.
- Engage with patients of all ages—including children in pediatric settings—and work with individuals with disabilities or developmental challenges to provide respectful and personalized care.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, assisted living facilities, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for medical documentation and record keeping.
- Strong knowledge of medical terminology, anatomy, physiology, aseptic technique, infection control practices—including infectious disease care—and medication administration protocols.
- Experience with specialized patient populations such as dementia or Alzheimer's care; memory care; behavioral health; pediatric or geriatric patients; hospice or palliative care; post-acute or sub-acute settings; home & community care environments; or working with individuals with disabilities.
- Ability to perform vital signs measurement accurately and respond swiftly to emergent situations using basic life support skills.
- Physical stamina to stand for extended periods and assist with patient transfers safely using appropriate equipment like Hoyer lifts or transfer belts.
- Excellent communication skills for effective collaboration within healthcare teams and compassionate interaction with patients and their families.
- Certification in Basic Life Support (BLS) is required; additional certifications such as phlebotomy training or specialized trauma experience are a plus.
