LPN (Licensed Practical Nurse)

Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in the client's HOME. As an LPN, you will play a vital role in supporting patients' health and well-being through direct care, case management, and effective communication with healthcare teams.

Duties

• Provide direct patient care, including administering medications and treatments as prescribed.
• Collaborate with healthcare professionals to develop and implement individualized care plans.
• Conduct patient assessments and monitor vital signs to track health status.
• Maintain accurate patient records and documentation in compliance with healthcare regulations.
• Support case management efforts by coordinating with multidisciplinary teams to address patient needs.
• Engage in hospital medicine practices, ensuring adherence to established protocols and standards of care.
• Foster a caring environment that promotes patient dignity and respect.
• Current LPN licensure in the state of South Carolina
• Experience in outpatient settings or long-term care preferred.

• Strong understanding of discharge planning processes and hospital medicine practices.
• Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare teams.
• Ability to manage multiple tasks efficiently while maintaining attention to detail.
• Must have experience with the use of tracheostomy, gastric or jejunostomy tubes, and indwelling catheters.

Pay: $33.00 - $37.50 per hour
