We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) with experience in Pediatrics to join our healthcare team. As an LPN, you will play a vital role in supporting patients' health and well-being through direct care, case management, and effective communication with healthcare teams.
Duties
- Provide direct patient care, including administering medications, feeding tubes, and treatments as prescribed.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Conduct patient assessments and monitor vital signs to track health status.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Support case management efforts by coordinating with multidisciplinary teams to address patient needs.
- Engage in hospital medicine practices, ensuring adherence to established protocols and standards of care.
- Foster a caring environment that promotes patient dignity and respect.
- Current LPN licensure in the state of South Carolina
- Experience in outpatient settings or long-term care preferred.
- Strong understanding of discharge planning processes and hospital medicine practices.
- Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare teams.
- Ability to manage multiple tasks efficiently while maintaining attention to detail.
- Must have a minimum of 2 years of experience with the use of tracheostomy, gastric or jejunostomy tubes, and indwelling catheters.
