About the Role
We are seeking a care management professional to provide whole-person care management in NC Medicaid Managed Care. Provider-based care management promotes integrated care and offers beneficiaries a choice in how they receive care management.
- Develop care management comprehensive assessments and care plans/individual support plans with beneficiaries
- Coordinate/refer/monitor all services (medical, pharmacy, behavioral health, waiver services, food, housing, transportation, community resource supports)
- Support beneficiaries in a crisis (with planning supports)
- Arrange for annual physicals
- Innovations and TBI waiver care coordination (if applicable)
- Convene and consult with a multidisciplinary care team
- Provide management for beneficiaries with chronic, high-risk, high-cost care management needs

Compensation
Competitive hourly pay aligned with your role, experience, and contribution, with opportunities for growth over time.
- Compensation: $23.00 - $29.53
Placement within the range is based on licensure level, education, and experience.

Responsibilities
- Maintain accurate and timely clinical documentation
- Collaborate with care teams as needed
- Maintain compliance with all clinical and regulatory standards
- Participate in supervision (if applicable based on licensure level)
Qualifications
- A bachelor's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area, or licensure as a registered nurse (RN)
- Two years of experience working directly with individuals with behavioral health conditions (if serving members with behavioral health needs) or with I/DD or a TBI condition(s) and
- For Care managers serving members with LTSS needs: two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above.
- A valid driver's license, auto insurance, and reliable transportation
