Essential Duties and Responsibilities:
- Collaborates with the Program Coordinator to complete interviews and acceptance into the program.
- Liaison between NCCF staff and other agencies ensure appropriate service linkages occur for medical care, mental health counseling, psychiatry, housing, employment, and other community providers.
- Ensures that each resident has a service plan which will allow them to successfully transition to stable housing within 12-18 months.
- Provides hands-on individual life skills training for FILP residents at minimum bi-weekly to address barriers that led to homelessness.
- Ensures client receives appropriate medical appointments, including annual physicals, annual vision, and bi-annual dental.
- Maintains contact with service providers to ensure continuity of care.
- Provides daily supervision by completing a minimum of one apartment check per shift to ensure safety and program compliance.
- Prepares bi-weekly contact notes, quarterly service plan documentation, psychosocial summaries and discharge summaries in a timely manner.
- Inputs information into government systems as required by FILP contract.
- Provides individual consultation to young adult clients regarding effective strategies to barriers toward implementing treatment plans.
- Assists young adult clients to develop skills and strategies for dealing with issues that have resulted in homelessness and/or unstable housing.
- Locates community resources for FILP clients, i.e. job/vocational training, higher education resources, summer camps/jobs, etc. and facilitates utilization of such resources by residents.
- Assists clients with accessing job and educational resources.
- Provide service coordination and assist with referrals for Foods Stamps, Medical Assistance, etc. if eligible
- Provides crisis and early intervention with youth struggling with pro-social behavior, adaptation, and acute mental health needs.
- Implements FILP program policies and procedures.
- Provides consultation to staff around cultural issues impacting the youth.
- Ensures that client files are maintained according to NCCF regulations

- Identifies gaps in services and advocates for increased resources.

Qualifications:
- Bachelor’s degree with two years of related experience preferred. Must have an understanding of the concepts of the life skills development, therapeutic milieu and the multi-disciplinary treatment team.
- Strong child care leadership with an ability to support and motivate a diverse staff serving vulnerable youth.
- Possession of strong interpersonal/conflict resolution skills. Ability to communicate effectively, both verbally and in writing, and to work as a team player.
- Broad working knowledge relative to the needs of the emotionally disturbed, victimized, learning disabled, delinquent adolescents and their family systems.
- A commitment to serving vulnerable youth who are challenged in transitioning to adulthood.
- An attitude compatible with the goals and purposes of the National Center for Children and Families.
