What are the responsibilities and job description for the RN Health Coach position at NBN GROUP?
The NBN Group is looking for Registered Nurses / RN licensed in New York or New Jersey with excellent interpersonal skills to join our team of Health Coaches.
The RN Health Coach utilizes a population health perspective to guide, care, and support our patients in their care transitions. Assess, evaluate, plan and implement care plans to assist our patients in reaching realistic health care goals. Collaborate with treating physicians, review the treatment plans and provide education to patients.
If you are looking for a rewarding, per diem work from home position, this is for you! We require at least 20 hours per week and up to 40 hours per week.
Qualifications:
- Must be a Registered Nurse in the State of New York or New Jersey
- Certificate in wellness or health coaching required.
- Able to build relationships with patients
- Knowledge of chronic disease management
- Excellent communication skills
- Must have terrific computer skills
