What You’ll Do

· Provide skilled nursing care in a home setting, focusing on patient education, recovery, and long-term health outcomes.

· Complete patient visits with a focus on quality, not quotas.

· Collaborate with a dedicated team to coordinate care and support patient progress.

· Maintain clear and accurate documentation of patient care and treatment plans.

What We’re Looking For

· Licensed RN in the state of Virginia.

· At least one year of nursing experience. in home health, medical-surgical, emergency department, or a skilled nursing facility. Home health experience a plus.

· Experience with IVs, wound care, and disease management.

· Ability to work independently while maintaining strong communication with the care team.

· Valid driver’s license and willingness to travel for patient visits and required meetings.

Schedule

· Monday through Friday, day shift.

· Rotating Weekend on call
