Full-Time Home Health RN

We are looking for a Full-Time Home Health RN who is comfortable traveling throughout the Madison / Greene County areas to provide care in patients' homes.

What You'll Do
- Provide skilled nursing care to patients in their homes throughout the Orange and Mineral County territory.
- Complete patient visits with a focus on quality, compassionate care.
- Educate patients and families regarding disease management, medications, treatments, and recovery.
- Collaborate with physicians and the interdisciplinary care team to coordinate patient care.
- Complete timely, accurate, and thorough clinical documentation.
- Monitor patient progress and communicate changes in condition appropriately.
- Develop meaningful relationships with patients while helping them remain safely in their homes.
What We're Looking For
- Current RN license in the Commonwealth of Virginia.
- At least one year of nursing experience in home health, medical-surgical, emergency department, or skilled nursing.
- Home health experience is a plus, but we are willing to consider strong candidates from other clinical backgrounds.
- Experience with IV therapy, wound care, and disease management.
- Ability to work independently while maintaining strong communication with the care team.
- Valid driver's license, reliable transportation, and willingness to travel throughout the Madison/Greene area.
- Strong clinical judgment, organization, and commitment to patient-centered care.

Schedule
- Monday–Friday, day shift
- Rotating weekend on-call

- Primary patient territory: Madison and Greene County
