Summary

This position is located in the CNRMA (N95) office within the CNRMA Fleet and Family Readiness Programs (N9) and reports directly to the Regional Operations Program Director. The primary purpose of the Care Manager is to ensure that Navy Wounded Warrior enrollees and their family members and caregivers are equipped to make a seamless transition from Active Duty into civilian life by taking goals to prepare the road for the reintegration phase of recovery process.

Duties

  • Case Management
  • Provides bedside support to Navy and Coast Guard recovering service members (RSM) and their families through minimizing delays and gaps in treatment or services. Obtains permission from RSMs to gather and exchange necessary information about the RSMs, family members, and caregivers in accordance with Privacy Act and Health Insurance Portability and Accountability Act (HIPAA) regulations.
  • Conducts comprehensive Initial Needs Assessments (INA) to identify immediate non-medical issues and collaborates with the RSM and their family to establish a framework for addressing those needs.
  • Works alongside Government and non Government agencies and service providers to facilitate case coordination, conduct outreach for program Awareness and assist Adaptive Recreation and Outreach specialist ( ARCO) and information sharing. Helps patient and family make informed decisions.
  • Coordinates with member Parent Command and Casualty Assistance Calls Officers to arrange invitational travel orders for family members, assisting with travel claims.
  • Consults and collaborates with multidisciplinary teams during initial treatment phase and continuing throughout the continuum of care.
  • Ensures that the RSMs, family, and caregivers have access to all medical and non-medical services including rehabilitation, education, employment-related programs, and disability benefits.
  • Initiates referrals to Department of Defense (DoD)/Federal, State, and local Governments and non-governmental organizations, intervening when necessary to ensure delivery of services.
  • Assists with Navy personnel specific issues such as orders, pay, housing, fitness reports/evaluations, awards and promotions, etc.
  • Provides information and referrals as necessary for the Sailor to make informed decisions regarding continuation on active duty. Documents uniformed member and family member information in program's electronic database of record and prepares required reports.
  • Responds to inquiries and provides information, guidance and assistance with information release approval from the Chain of Command.
  • Submits all required documentation on assigned cases via the program's system of record. Provides reports and updates on cases are required.
  • Program Support
  • Conducts regular outreach activities to educate the fleet, local commands, and medical treatment facilities about NWW services, thereby cultivating a robust network of referral sources.
  • Serves as the program's representative and advocates for RSMs in meetings, conferences, and other internal and external forums.
  • Analyzes case data for trends, prepares required reports, and shares findings with program leadership to assist in strategic planning/ marketing and policy updates.
  • Provides Navy Wounded Warrior Program event and activity support.

  • The incumbent must hole and maintain access to all relevant Wounded Warrior Systems and enrollee database to perform required duties.
  • Must be able to lift up to 50 LBS and stand for extended periods at a time at resource fairs and support recreation events in various weather conditions.

Knowledge and understanding of requirements and operations of DoD or Department of Navy (DON) Wounded Warrior initiatives, implementing congressionally and DoD mandated warrior and family/caregiver requirements.

Knowledge of Federal and agency regulations; knowledge of the specific organization's programs and missions, as well as knowledge of pertinent laws, regulations, and policies related to program/operations area to consider their impact on programs.

Knowledge of and skill in qualitative and quantitative methods for the assessment and improvement of program effectiveness and the improvement of complex management processes and systems, which have impact on Navy or NWW-SH goals and objectives.

Ability to manage several major projects concurrently, each of which require multiple tasks across many different levels of the Navy (e.g. local commands, regions, and headquarters) and other government agencies.

Ability to clearly and concisely prepare, present, discuss and defend issues, findings and recommendations.

Ability to work effectively on a team.

Ability to work in a stressful environment on time sensitive issues.

This position does not have a positive education requirement.

Salary is dependent on experience and/or education.

Some positions have special requirements. Selectee may be required to complete a one (1) year probationary period.
