Wound Care Nurse (RN or LPN)

Location: Flagstaff, Arizona
Schedule: Monday–Friday, 8-hour shifts (No Weekends)
Shift Options: 7am–3pm, 8am–4pm, or 8am–5pm

Job Summary

The facility is hiring one Wound Care Nurse, either an RN or LPN, to oversee wound prevention, wound management, ostomy care, and related clinical responsibilities for residents in a long-term care/skilled nursing setting. This role serves as a clinical expert and educator for nursing staff and collaborates closely with physicians and interdisciplinary teams.

Key Duties

• Perform wound assessments, reassessments, and ongoing wound care per policy and best practices.

• Develop and implement comprehensive skin care programs to reduce skin breakdown.

• Assist in developing and updating policies for wound care, wound prevention, ostomy care, tube/fistula management, and incontinence.

• Update and revise care plans based on resident response and overall wound healing progress.

• Assess and reassess resident pain; apply appropriate pain management strategies and educate residents/families.

• Provide in-services and training to nursing staff on wound prevention, wound management, ostomy care, and incontinence care.

• Communicate clearly and professionally with nursing staff and physicians.

• Collaborate with interdisciplinary teams to address resident needs holistically.

• Ensure all documentation meets current regulatory and practice standards.

• Attend training and continuing education to stay current on wound care best practices, advances, and new treatment modalities.

Requirements

• Active Arizona Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.

• CWOCN (Wound, Ostomy, and Continence Nurse) or CWCN (Certified Wound Care Nurse) certification required.

• Ability to obtain and maintain an Arizona DPS Fingerprint Clearance Card.

• Current CPR card obtained through a physical, in-person assessment.
