Position Summary

"Navitas Healthcare, LLC" is seeking an experienced RN Case Manager to coordinate patient care, manage utilization review, and support safe, timely discharge planning. Works closely with the interdisciplinary team to ensure appropriate level of care, accurate documentation, and effective communication with payers while meeting regulatory and quality standards.

Responsibilities

• Screen new admissions and identify discharge planning needs.

• Verify demographics/insurance and complete InterQual® assessments.

• Review admission appropriateness and ongoing level of care.

• Manage HINNs, appeals, and payer communication.

• Address psychosocial, financial, and legal barriers; make referrals.

• Conduct utilization review and respond to payer clinical requests.

• Collaborate with physicians and teams to ensure aligned care plans.

• Coordinate safe discharges and facility transfers.

• Monitor length of stay and escalate as needed.

• Support denial prevention and accurate reimbursement.

• Maintain compliance with policies, safety, and regulatory standards.

Qualifications

• 3 years recent acute care experience; case management preferred.

• InterQual® or similar UM experience required.

• Recent discharge planning & utilization review experience (within 4 years).

• Active MA RN license; BSN preferred.

• Strong communication, organization, and critical thinking skills.
