Job Title: Part-Time Licensed Master Social Worker (LMSW)
Location: New York, NY 10065
Duration: 27 Weeks
Schedule
- Variable shifts:
- Monday: 9:30 AM – 4:00 PM
- Tuesday: 9:00 AM – 5:00 PM
- Wednesday: 10:00 AM – 3:00 PM
- 24 hours per week (3x8 schedule across Monday–Wednesday)
Position Summary
We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide patient-centered social work services in a healthcare setting. The ideal candidate will assess patient needs, support care planning, and help connect patients and families with appropriate resources to enhance overall care outcomes.
Key Responsibilities
- Conduct interviews with patients and families to assess social, psychological, and environmental needs
- Review medical records and collaborate with care teams to determine appropriate interventions
- Provide individual and/or group counseling to patients and families facing personal or environmental challenges
- Connect patients and families with community resources, including financial assistance, counseling, rehabilitation, and medical support services
- Perform discharge planning, including coordinating placement in rehabilitation, extended care, or specialized facilities when needed
- Participate in interdisciplinary team meetings and communicate social and emotional factors affecting patient care
- Support care teams in addressing barriers to treatment and recovery
- Contribute to quality improvement initiatives and program development
Required Qualifications
- Minimum 1 year of experience as a Licensed Master Social Worker (LMSW)
- Strong knowledge of psychosocial assessments
- Experience in medical social work within a hospital or outpatient setting
- Valid Licensed Master Social Worker (LMSW) license issued by New York State Education Department (NYSED)
Education
- Master’s Degree in Social Work (MSW)
