Job Description
We are seeking an experienced Medical Social Worker (MSW) for a travel assignment in an acute care hospital setting. The ideal candidate will provide psychosocial support, discharge planning, and care coordination services for hospitalized patients while collaborating closely with interdisciplinary healthcare teams.
Key Responsibilities
- Conduct psychosocial assessments and evaluate patient support needs
- Develop and implement discharge planning and transition-of-care strategies
- Coordinate community resources, referrals, and post-discharge services
- Provide emotional support, crisis intervention, and patient advocacy
- Collaborate with physicians, nurses, case managers, and interdisciplinary teams
- Assist patients and families with care planning and resource navigation
- Maintain accurate documentation in accordance with hospital and regulatory standards
- Support safe and efficient patient discharge processes
Requirements
- Master’s Degree in Social Work (MSW) required
- Active California Social Work license required by start date
- Minimum 2 years of acute medical social work experience required
- Experience with both:
- Medical Social Work
- Discharge Planning
- Strong communication, care coordination, and patient advocacy skills required
- First-time travelers accepted with screening
