Job Summary
The Inpatient/ LTAC (Long Term Acute Care) Care Coordinator (ICC) plays an integral role in the “high risk” member’s journey towards better well-being. By serving as the link between members and appropriate health care personnel, the ICC is responsible for identifying the appropriate post-acute care setting ensuring efficient, smooth, and prompt health care services will be delivered to the member following the acute care episode. The position also provides emotional support to both the member and their relatives and provides medical advice during the discharge planning process.
Responsibilities
- Own the member relationship and is the single point of contact for hospitals with on-site engagement. On-site engagement defined as:
- EMR access
- Access to patients
- Engagement with hospital CM, D/C Planers, Social Workers, & Hospitalists
- Access to hospital census is preferred
- Own prospective skilled nursing facility requests that originate from an in-person request while on-site or from a call direct to the ICC personal line.
- Approve prospective requests within established parameters.
- Forward requests to utilization management partners if outside authorization parameters or if unable to complete the request within one hour.
- Perform other duties and responsibilities as required, assigned, or requested.
Qualifications
- Active, unrestricted registered clinical license required – Registered Nurse, Physical Therapist, or Occupational Therapist
- 3 - 5 years of clinical experience required
- At least 2 years of Case Management experience preferred
- Patient education background, rehabilitation, and/or home health nursing experience a plus
- Experience working with geriatric population preferred
- Exceptional verbal and written interpersonal and communication skills
- Strong problem solving, conflict resolution, and negotiating skills
- Proficient with Microsoft Office applications including Word, Excel and PowerPoint
- Independent problem identification/resolution and decision-making skills
- Detail-oriented
- Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously
- Team player
- Employment is contingent on satisfactory completion of pre-employment screening; this includes a background check, and additional screenings if applicable such as a drug test or tuberculosis test. Information regarding the screening process will be provided to you. The screening must be satisfactorily completed before employment date. Employment is contingent on you having, or obtaining within the required timeframe, any state and/or national certification that {UnitedHealth Group / Optum360} requires for your position.
Work Conditions and Physical Requirements
- Ability to adhere to all state, facility and company requirements, including vaccinations
- Ability to establish a home office workspace
- Ability to manipulate laptop computer (or similar hardware) between office and site settings
- Ability to view screen and enter data into a laptop computer (or similar hardware) within a standard period of time
- Ability to communicate with clients and team members including use of cellular phone or comparable communication device
- Ability to remain stationary for extended periods (1- 2 hours)
- Travel requirements
- Ability to mobilize to and within sites within an assigned local or regional market/area, including car transport, up to 85% of the time
