Position Summary
Under the supervision of the Patient Care Manager, the After Hours Nurse provides clinical nursing support during non-core business hours. This includes telephone assessments, home visits, and documentation, all in accordance with hospice standards and procedures.

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Position Summary
Under the supervision of the Patient Care Manager, the After Hours Nurse provides clinical nursing support during non-core business hours. This includes telephone assessments, home visits, and documentation, all in accordance with hospice standards and procedures.
Primary Responsibilities
- Uses clinical judgment and assesses needs of the patient and their families and initiates intervention to meet the immediate needs by utilizing principles of palliative care and following (but not limited to) the following guidelines:
- Assesses the need for skilled nursing by phone
- Makes home visits when necessary to stabilize patient conditions, deliver skilled care, or in the event of a patient death
- Communicates patient condition and concerns to the attending physician and implements new medication or treatment orders as instructed
- Notifies the Patient Care Coordinator of any complex or escalating clinical or administrative situations
- Makes routine/follow up calls or home visits to new admissions and current patients while on duty, utilizing backup system as needed, in accordance with Navian Hawai‘i's 24 hours per day 7 days per week service and admission policy
- Obtains and verifies updated patient information and physician orders by:
- Reviewing voicemail or reports from the RN Case Managers
- Reviewing patient medical records in the EMR system
- Communicating with the outgoing or incoming After-Hours Nurse
- Remains available by cellular phone during shift
- Clears calls with answering service within 10 minutes of receiving a phone message
- Documents all phone calls, home visits and medication orders in accordance with Navian Hawai‘i protocol
- Provides clinical report to Interdisciplinary Team by 7:00 AM detailing all patient interactions and interventions conducted during the shift
- Ensures any patient contact initiated within the final hour of the shift is documented and distributed before shift end
- May serve as the primary or backup on-call nurse in the absence of other After Hours staff
- Perform other related duties as assigned by the Patient Care Manager or Director of Clinical Operations
- Valid Registered Nurse License in the State of Hawai’i
- Bachelor of Science in Nursing (BSN) preferred
- 3 years of experience as a staff nurse in an acute hospital
- Hospice and/or home health nursing experience preferred
- A legally compliant background check
- Drug screen & physical exam
- Valid Hawai’i Driver’s License
- TB clearance
- CPR & BLS Certification
- Generally, indoors in-patient homes or Hospice Hawai’i homes, nursing, and acute care facilities; may include outdoors in all types of weather
