Position Summary:
We are seeking a skilled and compassionate NP/FNP to join our integrative healthcare team. You will provide patient-centered care, manage acute and chronic conditions, and collaborate with our multi-disciplinary team to deliver high-quality healthcare. This role is ideal for a provider who enjoys forming partnerships with patients to create individualized plans of care, and who values the integration of conventional and complementary therapies.
Responsibilities:
- Partner with patients and families to gather an in-depth history, conduct appropriate physical exams, and create patient-centered plans of care.
- Order diagnostic testing and/or referrals as appropriate to support each patient’s health goals.
- Prescribe medications, monitor their effects, and manage both short-term and long-term health concerns.
- Collaborate closely with colleagues and healthcare specialists to provide comprehensive, coordinated care.
- Educate patients on wellness, prevention, and treatment options, supporting their journey toward optimal health.
- Maintain accurate and detailed patient records in compliance with state and federal regulations.
- Foster a welcoming and supportive environment for patients, ensuring a positive healthcare experience.
- Stay current on medical advancements, evidence-based practices, and integrative healthcare trends through ongoing professional development.
