Job Description Summary
Coordinates and manages patient care and disposition planning for the Inpatient Psychiatric Unit.
Job Description
Essential Functions:
- Coordinates inpatient and post-discharge care, scheduling, and reporting.
- Provides disposition planning and maintains disposition data.
- Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
- Coordinates available NCH and community resources to enhance client care.
- Provides education and support to patients and their families regarding treatment options and resources.
- Completes documentation of discharge and care coordination activities in Epic and other related administrative paperwork in accordance with policy standards.
Bachelor's Degree in social work, counseling, psychology, or a related field, required.

Skills
- Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers and community representatives.
- Ability to work with the hospital’s electronic medical records system.
- Ability to work and problem-solve with minimal supervision.

- Spreadsheet, data entry and tracking, and computerized scheduling skills.
