Full Time Primary Care Physician opportunity
Pittsburgh, PA
Benefits:
Salary: Pay is dependent on experience up to $300k Yearly
Sign On Bonus: $30k

Primary Care Physician Specific Duties:
- Treat patients 16 and order age group.
- Develop strong transitional care plans and able to help keep track of paneled patients along the whole continuum of care including discharges from the hospital, ER, skilled nursing facilities/nursing homes, and home care.
- Treat complex, high-utilization patients with conditions like HIV, diabetes, hypertension, and substance use issues, focusing on "whole-person" care.
- Educate members on chronic disease progression and the need for advanced directives where applicable
Primary Care Physician Qualifications:
- Unrestricted PA Medical License
- Board Certified in Internal Medicine or Family Practice
- BLS certification
Preferred Primary Care Physician Qualifications:
- Experience in population health
- Experience with medically and psychosocially complex patients
