Nurse Practitioner – In-Home Health Risk Assessments (PRN)

Your Role:
As an NP, you’ll conduct comprehensive in-home HRAs for Medicare Advantage members. Each visit includes a review of medical history, medications, lifestyle factors, and social determinants of health. You'll also complete basic physical and mental health evaluations and provide preventive care education. No long-term care or follow-up is required.
Requirements:
- Active Nurse Practitioner license (all specialties except PMHNP)
- Willingness to travel within your county and surrounding areas
