We are currently seeking experienced Nurse Practitioners (NPs) and Physician Assistants (PAs) to conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Responsibilities and Requirements
- Conduct thorough in-home HRAs, gathering essential information about patients' medical history, medications, lifestyle, and social determinants of health
- Perform physical and mental health evaluations, documenting findings in real-time
- Deliver high-quality preventive care education, ensuring patients understand their health risks and how to mitigate them
