Key Responsibilities:
- Conduct in-home assessments and develop individualized care plans
- Administer medications, wound care, IV therapy, and other nursing procedures
- Monitor patient health and progress, and update care plans accordingly
- Educate patients and families on managing illness or injury and using medical equipment
- Coordinate with physicians, therapists, and other healthcare professionals
- Maintain accurate clinical documentation in accordance with agency policies and regulations
- Ensure compliance with state, federal, and home health regulations (e.g., Medicare/Medicaid)
- Participate in case conferences and performance improvement initiatives
- Licensed Registered Nurse (RN) in the state of Michigan
- 1-2 years of clinical nursing experience, preferably in home health, acute care, or hospice
- Knowledge of OASIS documentation is highly preferred
- Strong clinical, organizational, and communication skills
- Valid driver’s license and reliable transportation
- CPR certification (current and valid)
- Experience with electronic medical records (EMR) systems
- Ability to work independently with minimal supervision
- Adaptable and comfortable working in diverse home settings
