Position Overview
The RN MDS Coordinator is responsible for coordinating and completing resident assessments in accordance with applicable requirements and established timelines. You will collaborate with nursing, therapy, dietary, social services, and other departments to support accurate documentation and individualized resident care planning.
Key Responsibilities

Coordinate, complete, and document accurate resident assessments

Maintain the MDS assessment calendar and monitor required completion dates

Interview residents and family members to gather relevant clinical and care information

Collaborate with the interdisciplinary team to complete assessments, care area reviews, and individualized care plans
-
Help ensure documentation is accurate, complete, and submitted within required timeframes

Communicate assessment deadlines and follow up with department leaders as needed
-
Support quality outcomes and appropriate reimbursement through accurate MDS and PDPM processes
-
Participate in on-call responsibilities as needed
Qualifications

Current and active Registered Nurse license required

Strong organizational and time-management skills
-
Excellent written and verbal communication skills

Ability to collaborate effectively with an interdisciplinary team

Compassionate, dependable, and detail-oriented
-
Previous nursing experience preferred
-
MDS and PDPM experience preferred
-
RAC-CT, RAC, or CMAC certification is a plus but not required
Candidates with strong long-term care experience who are interested in growing their MDS knowledge are encouraged to apply.
