Position Summary
The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process, including MDS assessments, Care Area Assessments (CAAs), and individualized care plans in accordance with federal and state regulations.
This role partners closely with nursing, therapy, physicians, and the interdisciplinary team to ensure residents receive exceptional, person-centered care while maintaining compliance and reimbursement accuracy.
Qualifications

Active South Carolina Registered Nurse (RN) license

Graduate of an accredited school of nursing

Minimum one year of long-term care experience

Previous MDS Coordinator experience preferred

Working knowledge of Medicare, PPS, PDPM, and Managed Care reimbursement

Strong understanding of federal and state long-term care regulations
-
Excellent assessment, organizational, communication, and leadership skills
-
Experience with electronic medical record (EMR) systems
What You'll Do
-
Coordinate the interdisciplinary resident assessment and care planning process
-
Complete accurate and timely MDS assessments (admission, readmission, quarterly, annual, significant change, and discharge)

Complete Care Area Assessments (CAAs) and individualized care plans within required regulatory timelines
-
Review and update care plans to reflect residents' changing needs and goals
-
Lead and participate in care plan meetings and family conferences
-
Communicate resident care plan updates with families, physicians, and the interdisciplinary team

Manage Medicare PPS, PDPM, and Managed Care assessment schedules
-
Ensure clinical documentation supports assessment accuracy and reimbursement requirements
-
Assist with state and federal surveys and maintain regulatory compliance
-
Stay current on MDS regulations, reimbursement guidelines, and industry best practices
-
Educate staff on documentation and resident assessment processes
-
Maintain accurate resident records using electronic medical record (EMR) systems
-
Perform additional duties as assigned by the Director of Nursing
