🩺MDS LPN or RN Nurse– Full Time | Added to On-Call Rotation After 90 Days
🛏️ We are a 109-bed beautiful campus with skilled nursing and rehabilitation center. Â
Facility location: White Oak Manor York - 111 S Congress Street, York, SC 29745 - 803-684-0035Â Â Â
We’re seeking a dedicated LPN or RN Nurse Assessment Coordinator (NAC) to complete accurate and timely resident assessments, ensure compliance with regulations, and support quality, person-centered care. After 90 days, this role will be added to the on-call rotation. Â
Wage Scale Based on YOUR years of experience.
What You'll Do:Â
♦Complete and submit MDS and related documentation on time
♦ Conduct nursing assessments and collaborate on care plans
♦ Support nursing staff and participate in QA reviews and meetings
♦ Join the on-call rotation after training period
What We'll Need:
♦ Current LPN or RN license South Carolina
♦ Long-term care or MDS experience preferred
♦ Strong assessment, documentation, and teamwork skills
