What You'll Do:
♦ Lead and oversee the Social Services Department to support residents’ emotional and psychosocial well-being
♦ Develop and implement individualized care and discharge plans
♦ Partner with residents, families, and the care team to ensure needs are met
♦ Provide guidance, support, and crisis intervention to residents and families
♦ Coordinate referrals to community resources and services
♦ Ensure compliance with all federal, state, and facility regulations
♦ Maintain accurate documentation, care plans, and social histories
♦ Supervise and support Social Services staff
♦ Participate in interdisciplinary team meetings and quality assurance initiative
What You'll Need:
♦ Bachelor’s Degree in Social Work or related Human Services fieldÂ
♦ At least one (1) year of social services experience in a healthcare setting
♦ Strong communication and interpersonal skills
♦ Ability to lead, organize, and prioritize in a fast-paced environment
♦ Knowledge of regulatory requirements and resident rights
♦ Compassion, empathy, and a commitment to quality resident care
