What are the responsibilities and job description for the Emergency Room Community Navigator position at Nathan Littauer Hospital?
Job Purpose: As a member of the Care Coordination team, the Emergency Room Community Navigator is responsible for providing a full range of medical, social, and case management services to patients by providing an appropriate level of evaluation to determine/identify bio-psycho-social characteristics and social determinants of health needs of the patient and provide appropriate interventions for patients who are discharged from the Emergency Room as well as those patients admitted to the facility. To assist with patient/family and other hospital disciplines access to necessary services that extend the continuum of care beyond the hospital stay.
Education: Possess a Bachelor's degree or higher in Human Services or related field.
Experience: Possess one (1) year case management experience with diverse populations.
Preferred: Three (3) years case management experience.
Position Will Require Working One (1) Saturday Per Month
