Overview
Nassau County Council on Aging (NCCOA) announces an open Case Manager position in our Westside Senior Life Center in Hilliard starting immediately. This position provides services to assist Nassau County seniors and caregivers. We look forward to hearing from applicants who are professional, self-directed and have excellent communications skills, preference given to those with case manager experience and working the elderly population or a 4-year degree.

JOB SUMMARY:

The Case Manager is responsible for coordinating functions for Titles III-B, III-C1, IIIC2, Community Care for the Elderly (CCE), Home Care for the Elderly (HCE), Alzheimer’s Disease Initiative and Consumer Directed Care. Case Management is a client-centered service that assists clients in identifying physical and emotional needs and problems through an interview and assessment process; arranging and coordinating agreed upon services; and monitoring the quality and effectiveness of the services. Case management is a service for actively enrolled clients that provides continuing support and addresses the changing needs of clients.

DUTIES AND RESPONSIBILITIES:

Conduct assessments and create care plans to identify, evaluate and address the client’s continuing and changing needs.

Link clients with appropriate formal and/or informal support systems necessary in order to improve the client’s circumstances.

Provide linkage between health care and social service delivery systems. This requires involvement with physicians, hospitals, health maintenance organizations (HMOs), nursing homes and health services.

Actively pursue the development of informal resources to help meet the client’s needs.

Assist clients in scheduling appointments for various services.

Provide EHEAP (Emergency Home Energy Assistance Program) application assistance to clients.

Assume responsibility for preparing eligibility lists of senior citizens who qualify for sewer, refuse and landfill exemption for the City of Fernandina Beach.

Assist clients in the town of Hilliard with water bill exemption payments.

Thoroughly read and periodically review the Department of Elder Affairs (DOEA) Case Management Manual and become familiar with definitions, services and terms.

Arrange training for the client’s family members, relatives and friends in methods of caregiving, if needed.

Monitor services to ensure they are having a positive impact on the problems that necessitated the service.

Development and maintain an accurate and updated file on community resources within Nassau and bordering counties.

Attend community meetings with sister agencies to ensure that the latest information in community resources is being communicated.

Maintain a complete updated file on each client and include supporting documentation as required by the Agency for Healthcare Administration (AHCA). Keep detailed, accurate records on clients.

Keep files locked in secured file cabinet at all times.

Responsible for reporting all client changes in status for input into CIRTS upon receipt. Case manager must collect, review and summarize total units of service provided for weekly input into CIRTS system.

Assist in the preparation and execution of evaluation reports.

Make home visits and telephone contacts as required in the DOEA handbook.

Provide routine information and referral services to the public at large through telephone interviews and walk-ins.

Attends meetings, conferences and workshops as directed by the Program & Services Director.

Conduct all quarterly and annual assessments on clients as mandated by DOEA.

Assist in developing plans for service delivery system.

Conduct outreach services throughout the year at Health Fairs and other outlets.

Assist in developing press releases in conjunction with NCCOA activities.

Prepare and submit month-end and year-end reports, as requested by Program & Services Director.

Secure appropriate needs assessment materials, distribute same and analyze data for program application and input.

Contact Elder Abuse Hotline immediately if client abuse or neglect is suspected.

Perform other NCCOA related duties as directed by Program & Services Director.

KNOWLEDGE, SKILLS AND ABILITIES:

The case manager’s role is that of “gatekeeper” in the community care system. Therefore, the case manager must be knowledgeable about the array of community-based services and resources available to address the needs of clients and their caregivers.

Must have knowledge of obstacles facing the aging and vulnerable population.

Excellent verbal communication skills.

Excellent writing skills.

Microsoft office skills (word, excel, power point, etc.).

Professionalism

SUPERVISION:

Performs duties under the supervision of the Program & Services Director.

WORK EXPERIENCE/REQUIREMENTS:

Two (2) years case management experience preferred.

Experience working with the elderly or other vulnerable population in a customer focused environment.

Must be able to pass the Level 2 background check required by DOEA.

Must adhere to Drug Free Workplace standards.

Must have use of personal vehicle for outreach and home visiting purposes.

Must have a valid driver’s license.

EDUCATION:

Bachelor’s degree in social work, psychology, sociology, nursing, gerontology or related field. Year for year related job experience or any combination of education and related experience may be substituted for a bachelor’s degree upon approval of ElderSource.

PHYSICAL WORK REQUIREMENTS:

Able to sit and work at a computer keyboard for extended periods of time.

Able to stoop, kneel, bend at the waist and reach on a daily basis.

Able to perform general office administrative activities: copying filing delivering and using the telephone.

Able to lift 30 pounds.

Pay: $38,000.00 - $40,000.00 per year
