• Social Services Director in identifying and providing for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential during his/her stay at the facility and to assist in the planning for his/her discharge.
  
  
  

Responsibilities:

• Assisting in collecting social history data and in the development of a comprehensive discharge plan, including the MDS and Care Pan for new admits.
• Document progress notes, which relate to each resident's care plan when necessary.
• Provide timely and accurate completion of Social Services portion of the MDS, Resident Assessment Protocols, Resident Care Plan, and Progress Notes as well as Social Services Assessment, and other forms as required.
• Function independently, and have flexibility, personal integrity, and the ability to work effectively.
• Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Director.
  
  
  

EDUCATION: Bachelor's degree in a Health and Human Services field.
