HOUSING COORDINATOR/CASE MANAGER:
The successful applicant will demonstrate self-motivation, strong interpersonal skills, an aptitude for problem solving, a passion for social justice, and the ability to help program participants secure sustainable housing.
POPULATION SERVED: Institutionalized individuals with serious mental illness, who are homeless or at risk of homelessness. Institutions include behavioral health facilities, hospitals, Department of Corrections, state hospitals, and other institutions.
JOB DESCRIPTION: To provide assistance to persons transitioning to community housing recovery while overcoming barriers to obtaining and maintaining housing and appropriate supports.
SOME DUTIES/RESPONSBILITIES INCLUDE BUT ARE NOT LIMITED TO:
- Research, develop, and update comprehensive housing resource database of affordable housing availability
- Establish and maintain contracts with residential programs.
- Conduct site visits, ensuring safety of housing, and satisfaction of the individual(s) residing at the facility
- Maintain a caseload of individuals by conducting and documenting face to face visits with client(s), at residential sites and connecting clients with needed resources
- Assisting clients with social security applications using the SSI/SSDI, Outreach, Access, Recovery (SOAR) model, SNAP benefits, permanent supportive housing applications, Medicaid, and other mainstream benefits as well as scheduling doctor appointments/transportation
- Maintain budget and track outcome data, create various reports
- Participate in employer's activities (conferences, outreach, trainings, meetings, webinars, presentations, etc.)
- Other duties/responsibilities as assigned by the Executive Director

Pay: $39,520.00 - $41,600.00 per year
