Role Description
This is a full-time hybrid role for a Hospice and Home Health Liaison based in Littleton and the surrounding areas. The Liaison will be responsible for building relationships with healthcare professionals, community partners, and families to promote hospice and home health services. Responsibilities include providing education on available care options, conducting outreach efforts to grow referrals, coordinating with care teams to ensure smooth transitions for patients, and serving as a trusted resource for individuals navigating their care journey.
Qualifications
- Knowledge and experience in Hospice Care, End-of-Life Care, and Palliative Care
- Familiarity with Home Care services and the healthcare industry
- Strong skills in Sales and relationship-building
- Excellent interpersonal and communication skills
- Ability to work both independently and collaboratively in a hybrid work environment
- Commitment to supporting patients and families with compassion and competence
- Previous experience in hospice, home health, or related fields is highly desirable
- Bachelor’s degree in healthcare, social work, marketing, or related field preferred
- Valid driver’s license and reliable transportation for travel within the service area
