Director of Admissions – Home Health & Hospice Position Summary The Director of Admissions is responsible for leading and overseeing the entire patient intake process for both home health and hospice services. This role ensures timely, accurate, and compliant admission workflows while delivering exceptional experience for patients, families, and referral partners. The Director of Admissions works alongside the intake team and serves as a key liaison between intake, clinical operations, and marketing. This leader drives process improvement, monitors compliance, supports patient advocacy, and actively participates in the intake workflow to ensure operational excellence. This leader is hands on and also participate in intake, not just supervising. The compensation range for this position is $90,000 - $110,000. Key Responsibilities Leadership & Team Oversight · Supervise and support Intake Coordinators and Lead Intake Coordinator · Collaborate closely with the clinical admission team (with clinicians reporting to the Director of Clinical Services) · Provide coaching, training, and performance management for intake staff · Foster a culture of accountability, efficiency, and patient-centered care Admissions & Intake Operations · Actively participate in and support the intake process alongside the team · Oversee referral intake, insurance verification, and data entry accuracy · Ensure timely coordination of clinical assessments and admissions · Maintain smooth and efficient transitions of care for all patients Referral & Relationship Management · Build and maintain relationships with hospital discharge planners, physicians, and community partners · Serve as a liaison between intake, marketing, and clinical teams · Support growth through strong referral management and partnership development Performance Monitoring & Process Improvement · Track and analyze conversion rates for home health and hospice admissions · Lead census-driven and admission-focused QAPI initiatives · Report weekly admission and intake performance metrics · Identify workflow gaps and implement process improvements Regulatory Compliance · Ensure all intake processes and documentation meet Medicare, Medicaid, state, and accreditation standards · Monitor compliance risks and implement corrective actions as needed · Maintain accurate records and ensure proper documentation standards are followed Patient & Family Experience · Serve as a key point of contact for patients and families during the admissions process · Provide education on levels of care, services, and patient rights · Support patient advocacy and ensure a compassionate, seamless onboarding experience Additional Responsibilities · Track and manage facility contracts · Monitor trends and risks identified through audits or intake reviews · Support organizational initiatives related to growth, quality, and compliance Core Intake Responsibilities · Manage incoming referrals from hospitals, physicians, and community partners · Monitors response times on referrals, and provides education, guidelines, and process improvement to ensure targets are being met. · Verify insurance eligibility, benefit, and obtain required authorizations · Accurately enter demographic, clinical, and diagnostic information into EMR systems · Coordinate initial visits (nursing, therapy, aide services) · Educate patients and families on services and expectations Qualifications Required / Preferred Credentials · Preferred: RN (Registered Nurse) or LPN (Licensed Practical Nurse) · Alternatively: strong background in intake coordination, admissions, or healthcare marketing Experience & Skills · Strong knowledge of home health and hospice regulations · Experience with EMR systems and insurance verification processes · Understanding of Medicare/Medicaid requirements · Proven ability to build and maintain referral relationships · Strong leadership, organizational, and communication skills · Ability to manage multiple priorities in a fast-paced environment Location Requirement · Must reside in the Denver Metro Area · Position is based out of the Namaste Health office What Success Looks Like · Efficient, compliant, and patient-centered admissions process · Strong collaboration across intake, clinical, and marketing teams · Consistent growth in referrals and admissions · Improved conversion rates · High team engagement and performance · Proactive identification and mitigation of compliance risks The employer for this position is stated in the job posting.
