Overview
Namaste Adult Daycare is a community-centered Adult Day Health program dedicated to supporting older adults and adults with disabilities through compassionate care, meaningful activities, and culturally inclusive services. We strive to promote independence, social engagement, and overall well-being in a safe and welcoming environment.
We are seeking a Social Services Coordinator to join our team. This position plays an essential role in supporting participants’ social and emotional needs, assisting families, coordinating services, and ensuring compliance with all required documentation and Department of Health standards.
Responsibilities
- Conduct participant intake assessments and gather psychosocial information.
- Assist participants with social, emotional, and adjustment-related concerns.
- Participate in interdisciplinary care plan meetings and contribute to goal-setting.
- Connect participants and families to community resources such as transportation, benefits programs, housing assistance, and senior services.
- Maintain accurate, timely documentation including progress notes and care plan updates.
- Serve as a primary point of contact for families and caregivers, providing updates and support.
- Observe and report changes in participant behavior or needs to clinical staff.
- Assist with daily program operations, participant engagement, and special events.
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field (preferred).
- Experience working with older adults, adults with disabilities, or in a community health setting.
- Strong communication, interpersonal, and organizational skills.
- Ability to maintain accurate records, manage documentation, and uphold confidentiality.
- Compassionate, patient, and able to work collaboratively within a team.
- Bilingual in English and Vietnamese is preferable.
Pay: $60,000.00 - $70,000.00 per year
