Role Description
This is a full-time, on-site Case Manager role based in Nashville, TN. The Case Manager will provide individualized support to human trafficking survivors, helping them navigate resources for recovery, housing, education, legal aid, and employment opportunities. Responsibilities include facilitating various groups, developing personalized care plans, collaborating with community partners, linking clients to needed services, and maintaining accurate documentation. The Case Manager will directly engage with clients to assist them in achieving self-sufficiency, while working in close coordination with the NAHT team and partner organizations. Empathy, professionalism, and organizational skills are essential to this role.
Qualifications
- Experience in case management, social work, or similar fields
- Strong interpersonal skills and the ability to build trust with diverse individuals
- Proficiency in creating individualized care plans and coordinating services
- Excellent organizational, time management, and documentation skills
- Ability to collaborate with other team members, agencies, and organizations
- Familiarity with issues surrounding human trafficking, trauma-informed care, and survivor advocacy
- Proficiency in computer skills, including word processing, spreadsheets, and case management software
- Bachelor’s degree in Social Work, Counseling, Psychology, or a related field (preferred)
- Experience working with trauma survivors or marginalized populations (a significant plus)
