NAFI Connecticut, Inc. (NAFI CT) is one of Connecticut’s most respected child and family serving agencies, with program locations across the state. We are seeking a Part-Time Licensed Practical Nurse (LPN), approximately 20 hours per week, to join a short-term residential program providing compassionate, trauma-informed care for young women ages 12–17. The program provides comprehensive clinical, therapeutic, educational, medical, and case management supports designed to promote each young person’s health, safety, stability, and well-being while supporting successful transitions to family, caregivers, or other appropriate community settings. The Part-Time LPN will work under the direction of an RN and alongside the program’s Medical Services Coordinator, clinical team, educators, case managers, and residential staff. At NAFI CT, you’ll be part of a team of compassionate, skilled, solutions-oriented professionals who are unwavering in their commitment to the children and families in our care.

Responsibilities Include: 

  • Provide selected practical nursing services under the direction of an RN or APRN and in accordance with provider orders, agency policies, and applicable requirements
  • Support nursing intake procedures, health screenings, and the collection and reporting of health information when youth enter the program and throughout their stay 
  • Respond to routine health concerns, recognize changes in condition, and promptly communicate or escalate concerns to the RN, APRN, or other appropriate provider 
  • Support safe medication administration and medication management practices, including medication reconciliation and accurate documentation 
  • Assist with scheduling and follow-up for primary care, dental, vision, specialty, and other community health services
  • Maintain complete, timely, and accurate electronic health records
  • Provide practical health education and guidance to youth, families, caregivers, and staff 
  • Collaborate with families, caregivers, community providers, and program staff to promote continuity of care and successful transitions

       Desired Qualifications:

      • Graduate of an accredited/state-approved practical nursing program and current, unrestricted LPN license in the State of Connecticut
      • Fluency in English and Spanish, with the ability to explain health information clearly and respectfully in both languages
      • A caring, dependable, and team-oriented approach to nursing 
      • Strong organization, communication, and documentation skills
      • Comfort working with adolescents in a collaborative residential setting 
      • Ability to work an approximately 20-hour-per-week schedule, with occasional flexibility for backup consultation or coverage based on program needs
      • Experience: Previous direct nursing experience is preferred; new LPN graduates are encouraged to apply
      • Experience with adolescents, behavioral health, or residential programs is welcomed but not required

      Salary/Hourly Rate: $35.00 per hour
