What are the responsibilities and job description for the Family Advocate position at NAFI Connecticut / Rhode Island / New York?
NAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We work with adults, children, and families to increase communication and social skills, improve relationships, increase circles of support, and provide therapeutic services based on best-practice research. We believe that people can change, and we work to empower each individual in their own care.
We are currently exploring candidates for a Family Advocate in our LYNC program. LYNC is a center-based program for court -involved youth and their families. Specific services include comprehensive assessment, crisis stabilization and brief individual therapy, case management, skill building groups, educational and vocational support, and fun family - strengthening activities.
Responsibilities Include:
- Provide center-based and community based behavioral services to juvenile justice youth and their families
- The Family Advocate in the point of contact for youth and families, service delivery in the program and connections to the community
- Conducts intakes, screenings and assessments
- Provide structured case management, work on goals developed, address barriers and develop and build skill acquisition
- Work closely with the Service Specialist to connect youth and families to community services and/or group interventions
- Work collaboratively with juvenile probation and provide enhanced family-based services
