The Opportunity:
If you are a dynamic, motivated individual who is driven to create healthier communities through excellent patient care, Saint Luke's has a fantastic opportunity for a full-time registered nurse in our in-patient environment. BSN Degree required within 3 years of start of position.
Shift: Full time Days
Job Description:
Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. Care Connections Nurse will work with the patient to focus on collaboration between hospital and community-based providers and arranging post discharge services. Care Connections Nurse functions as a member of a multidisciplinary team with a goal of improving the patient's health status and reducing the need for hospital care. Duties may also include assistance with program development, evaluation, professional education, or supervision of students.
