Job Summary:

Under appropriate supervision and in accordance with the Texas Nursing Practice Act (NPA), the Licensed Vocational Nurse (LVN) applies the nursing process to perform focused assessments, participate in planning and implementing nursing care, and evaluate responses to interventions. The LVN ensures patient safety and well-being through collaboration, accurate documentation, and safe clinical practices.

Essential Duties and Responsibilities:

• Collect data and perform focused nursing assessments of individual health status.

• Participate in planning nursing care based on identified patient needs.

• Assist in the development and modification of individualized nursing care plans.

• Engage in health teaching and counseling to promote, attain, and maintain optimal health.

• Assist in evaluating patient responses to nursing interventions and identifying further care needs.

• Administer medications and treatments safely, understanding rationale and potential effects.

• Accurately and completely document and report administration of medications and treatments.

• Communicate and document patient responses to prescription and nonprescription interventions.

• Report and document untoward reactions or adverse effects of medications, treatments, or procedures.

• Use evidence-based information to contribute to interdisciplinary policy development related to a safe clinical environment.

• Clarify questionable orders or treatment regimens with the appropriate licensed practitioner.

• Implement safety measures to protect clients, families, and healthcare team members.

• Obtain proper supervision and instruction when implementing unfamiliar procedures or practices.

• Seek training and orientation when using new equipment or addressing unfamiliar care situations.

• Notify appropriate supervisory personnel when leaving a nursing assignment.

• Maintain appropriate professional boundaries within the nurse-client relationship.

• Comply with Texas mandatory reporting laws under the Nursing Practice Act, including reporting:

  • Practice violations contributing to serious injury or death

  • Suspected impairment due to substance use

  • Abuse, exploitation, fraud, or professional boundary violations

  • Lack of competency, knowledge, or judgment posing harm to others

  • (Excludes minor incidents as defined by board rules)

• Provide nondiscriminatory nursing care, regardless of client demographics or conditions.

• Take appropriate precautions to prevent exposure to infectious or communicable diseases.

• Maintain continuing competence in nursing practice and commit to ongoing professional development.

• Accept nursing assignments only when client safety can be ensured and the nurse has adequate preparation and capacity.

• Perform other nursing acts as permitted by board rules, based on training, experience, and demonstrated competencies.
