What are the responsibilities and job description for the Transportation Program Navigator position at Na Hoaloha-Maui Interfaith Volunteer Caregivers?
About us
Na Hoaloha is a small non-profit business in Wailuku, HI. Our goal is to provide free supportive services to home bound older adults through community volunteers.
Kūpuna are living treasures of our community, keepers of cultural wisdom, and valued members of our society, yet many kūpuna continue to be underserved by local support systems designed to help us thrive as we age. Home and community-based services have been curtailed since the pandemic, putting a heavy strain on kūpuna and caregivers to piece together needed services to successfully age-in-place in our homes and communities.
We are looking for an experienced, well-organized, strong multi-tasker with a friendly, upbeat personality and willingness to learn. Must have at least one year of experience working in transportation, human services or a healthcare office setting. We seek the right fit to round out our friendly team of caring volunteers and caregivers.
Caring for kupuna is one of society's most essential tasks, yet a fragmented healthcare system misses opportunities to ensure that our aging population is supported across the continuum of care. Kūpuna face daily challenges including chronic health issues, poverty, and access to food and social services. We aim to provide older adults with care, support, and respect. Needs are supported through transition of care support, health education, chronic condition management, and social services assistance through a culturally relevant approach.
Position oversees multiple transportation programs with integration and coordination with volunteer operations, while organizing service delivery for older participants. Position handles all aspects of our extensive transportation network.
Answer phones, coordinate participant transportation requests, determine eligibility and assessment for program services, and process ride requests.
Process request follow-up for the participant with appropriate volunteer match. The main responsibility of the position is to handle the day-to-day transportation operations thru our network of trained volunteers.
Works with agencies and other volunteer organizations to support program services and resources.
Highly detail oriented with the ability to prioritize tasks independently, provide accurate and timely responses to participant, volunteer, and agency service requests.
