myPlace Health
Los Angeles, CA Full Time
POSTED ON 4/9/2026
AVAILABLE BEFORE 5/8/2026
myPlace Health is built around a simple but powerful belief: older adults deserve the support they need to live safely, independently, and with dignity in their own communities. As a PACE (Program of All Inclusive Care for the Elderly) organization backed by SCAN Group, myPlace Health brings together customized medical care, social activities, and daily support for participants and their families – all under one roof.

Our centers are more than healthcare facilities. They are vibrant community hubs where participants are known by name, valued, and supported as whole people. Behind that experience is a dedicated, interdisciplinary team working together to coordinate care, remove barriers, and improve quality of life for some of the most medically and socially complex populations.

For employees, myPlace Health offers the opportunity to do deeply meaningful work in a highly collaborative setting. Team members are encouraged to contribute innovative ideas, and grow alongside a mission that prioritizes compassion, respect, and impact. The result is a culture where people feel connected—to their colleagues, their participants, and the communities they serve.

At myPlace Health, work is more than a job. It’s a shared commitment to honoring what matters most.

Are you passionate about making a meaningful impact in the lives of the aging community by delivering personalized occupational therapy that enhances their independence and well-being? Do you thrive in a collaborative environment, working closely with an interdisciplinary team to create and implement individual care plans? Then we would love to have you on our team!

In this role, you'll have the opportunity to deliver personalized occupational therapy to PACE participants in various settings, including our care center, their homes, and even virtually. Your work will make a meaningful impact as you provide occupational therapy that meets each participant's unique needs, whether they require acute care, restorative support, or ongoing maintenance.

As a valued member of the interdisciplinary team (IDT), you'll collaborate closely to create and implement individualized care plans that truly make a difference in the lives of our participants. Partnering with our dedicated Physical Therapist, you'll also play a key role in ensuring the smooth operation of the rehabilitation department at our PACE center.

We’re excited to welcome a passionate team player like you who is committed to enhancing the well-being and independence of the elderly adults we serve.
