Role Description
This is a contract hybrid role hiring in multiple locations offering the flexibility to work both on-site and remotely from home. The Case Manager will be responsible for managing care coordination and communication between clients, medical providers, and other stakeholders. Key responsibilities include assessing client needs, developing and monitoring care plans, facilitating medical treatments, ensuring compliance with protocols, and providing consistent support to clients. This role requires strong organizational and interpersonal skills to ensure seamless case management and serve the best interests of the clients.
Qualifications
- Proficiency in care coordination, healthcare guidance, and understanding of the workers' compensation process
- Strong organizational, planning, and documentation skills to ensure efficient and effective case management
- Exceptional communication, collaboration, and interpersonal skills for working with diverse clients, medical professionals, and stakeholders
- Ability to work independently in a hybrid role and handle cases with empathy and professionalism
- Prior experience or relevant certification in nursing or case management preferred
- Knowledge of local healthcare systems, providers, and resources is a plus
Requirements:
- Degree in Nursing (RN)
- CCM (Preferred)
- Worker’s Compensation Experience - a PLUS!
- Catastrophic Case Management Experience - HUGE PLUS!
- Independent and Entrepreneurial – Nurses must be able to handle a case load from start to finish with minimal upline involvement.
- Comfortable with Electronic Communications (text, email, etc.)
- Quick Communications
- Experience with Microsoft Office products
