Job Summary for Population Health Coordinator: The Population Health Coordinator (internally known as a Value Based Medicine Specialist) is an expert in value-based primary care measures, care gap outreach and closure guidelines. This role is responsible for phone outreach to patients for care gap management, medication adherence, suspect condition assessment, and communicating critical information to stakeholders. The ideal candidate must be very comfortable with outbound phone communication.
Qualifications for a Health Population Specialist:
- 1( ) year(s) of experience in a value-based primary care setting managing care gap closure for large quantities of patients
- Microsoft Excel and EMR experience proficiency required; (Epic EMR a plus, but not required)
- Possess strong verbal and written communication skills
- Excellent interpersonal and customer service skills
- Bilingual in English and Spanish preferred
Job Type: Full-time
Pay: $19.00 - $24.00 per hour
Expected hours: 40.0 per week
