Location: Central Massachusetts (Worcester County and surrounding areas)
Job Type: Independent Contractor / Home-Based
Pay Rate: This is a bi-weekly stipend-based arrangement determined by the Massachusetts Department of Developmental Services (DDS) based on the level of need of the individual supported. Stipend amounts vary accordingly.
Job Description:
My Choice Programs is seeking caring and reliable individuals, couples, or families to become Shared Living Providers. In this role, you will welcome an adult with a disability into your home and provide daily support while promoting independence and community involvement.
This is a unique, home-based opportunity to make a meaningful impact in someone’s life while receiving ongoing support.
Responsibilities:
- Assist with activities of daily living (ADLs) such as cooking, hygiene, and routines
- Provide companionship and encourage social/community participation
- Support household tasks including grocery shopping, cleaning, and laundry
- Help with medication reminders and attending medical appointments
- Assist with basic budgeting and daily structure
- Provide reliable transportation to appointments and activities
Required:
- Valid driver’s license, reliable vehicle and a good driving record
- Ability to pass CORI and fingerprinting background checks
- Must have a spare bedroom available in your home
- Willingness to obtain First Aid/CPR certification
- Compassionate, patient, and dependable
- Must reside in Massachusetts (Per Regulations from the Massachusetts Department of Developmental Services)

Additional Information:
