Prior working exp. utilizing strong communication skills (written and verbal). Required

Prior experience working in a Team environment. Required 1 Years
Prior exp. working w/at risk youth. Highly desired
Prior working experience in a community or State program. Highly desired
Prior experience working in a youth educational program. Highly desired
Job Responsibilities may include, but not limited to:
- Facilitate the implementation of a life skills curriculum, titled Next Steps.
- Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
- Evalute the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.
- Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.
- Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.
- The Transistion Coordinators will represent the Indiana Department of Correction- Division of Youth Services at many conferences and meetings.
- The Transistion Coordinators will assist in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University’s HOPE Mentoring.
- The Transition Coordinators will assist in the development of the individiualized Education Learning Plan for each youth upon entry at the facility.
- Work with state, county and private agengies to provide re-entry servicers for youth transitioning back into the community.
- Assist students with obtaining social security cards, state identification, driver’s license, birth certificates, and additional documentation.
- Assist students with transitioning back to public schools
- Assist students with employment, apprenticeship programs, and military service information
Job Type: Contract
Pay: $20.00 - $21.00 per hour
Expected hours: 40 per week
