What are the responsibilities and job description for the COFR CASE MANAGER position at Muskegon County?
The COFR Case Manager, under general direction and supervision of the COFR Specialist, provides comprehensive community services for out-of-county individuals with mental illness, intellectual/developmental disabilities, youth, and autism disorder. They will have knowledge and understanding of specialized Adult Foster Care (AFC) homes while providing assessments, treatment planning, coordinating, monitoring, review, and advocacy for out-of-county consumers. They will also provide supportive services, crisis intervention, and mental health-related education for consumers, families, guardians/ payees, and significant others while completing, coordinating, obtaining and monitoring documentation of services provided.
1. Possess a high school diploma or certificate of completion of the General Education Development (GED) test; AND have three (3) years of experience in providing supports and/or services to persons with mental illness, developmental disabilities or substance use disorders;OR
Possess a bachelor’s degree in human services field, such as, psychology, social work, human services, etc.
2. Possess a valid driver’s license and the ability to obtain a Michigan Driver License within 30 days of employment.
3. Must have reliable transportation that may have to be used to carry out job duties of this classification.
Preferred but not Required
1. Some post high-school education is preferred, but not required.
2. Bilingual candidates highly desired.
3. Lived experiences with mental illness/developmental disabilities/substance use disorders valued.
4. Individuals in Recovery and individuals with experience in Armed Services valued.

PHYSICAL ACTIVITIES
An employee in this class performs generally sedentary work requiring the occasional lifting of objects weighing thirty-five (35) pounds or less.
ENVIRONMENTAL CONDITIONS
An employee in this class generally works in the offices of HealthWest, although travel to other County locations and out of the County travel will be required.EVALUATION CONTENT
Applicants for County employment may be subject to a post-offer criminal background check, depending on the nature and assignment of the position. When such check is conducted, an offer of employment is contingent upon the results of the criminal background check. Appointees must satisfactorily complete a probationary period before the appointment will be considered permanent.
