Reports to:
School Principal(s)
System Lead Nurse
Minimum qualifications: current LPN license and CPR certification
Responsibilities:
Consults with, conducts in-service training for, and serves as a resource for staff and administrators.
Develops and implements individual health care plans for students with special health care needs in collaboration with student, family, staff, and health care providers.
Identifies students enrolled or who plan to enroll in school that require or will require health care services at school.
Identifies students requiring or potentially requiring emergency management plans and designs and implements the plans in collaboration with students, families, other school staff, and community agencies as appropriate.
Serves on Individual Education Program committees for special education students and assists in the development and implementation of Individual Education Plans (IEPs) for students with special health care needs.
Serves as case manager for students with identified health needs.
Establishes a reporting and recording system that documents pertinent student health information.
Implements and/or supervises health program activities such as vision, hearing, spinal, height, weight, asthma, and tuberculosis screenings. Responsible for medication administration and documentation, immunization assessment, and follow-up.
Monitors compliance with immunization law.
Assesses and manages communicable health problems in the school population such head lice, scabies, and impetigo.
Assists school personnel in determining the need for exclusion and readmission of students in connection with infectious and contagious diseases.
Completes other duties as assigned.
