Required Qualifications
- Advanced Practice Provider: Nurse Practitioner (NP) or Physician Assistant (PA)
- Infectious Disease experience preferred, but motivated new graduates with strong clinical training are encouraged to apply
- Strong clinical assessment and diagnostic skills
- Passion for delivering patient-centered care in a collaborative team environment
- Commitment to evidence-based medicine, infection prevention, and quality improvement
- Provide inpatient and outpatient Infectious Disease follow-up care
- Evaluate and manage patients with acute and chronic infectious conditions including sepsis, bacteremia, osteomyelitis, hepatitis, and complex infections
- Collaborate with physicians, pharmacists, nursing staff, and care management teams to develop individualized treatment plans
- Assist with antimicrobial stewardship initiatives and infection prevention strategies
- Participate in patient education, transitions of care, and coordination with referring providers
- Utilize Epic EHR and integrated clinical tools to support efficient, high-quality patient care
