Hospice RN Case Manager – Home Hospice
Location: Barre, VT 05641
Facility: Central Vermont Home Health & Hospice
Contract Length: 20 Weeks
Shift : 12HR Days
36HR/Week
Travel RN Rate: $72.7/hour
Estimated Weekly Gross: $2617.2/week
Taxable: $828/week
Non-Taxable: $1789.2/week
Local RN Rate (within 50 miles): $68.00/hour
Central Vermont Home Health & Hospice is seeking experienced Home Health RNs to provide individualized, goal-oriented nursing care to patients in their homes. The RN will provide care according to physician orders and may serve as the case manager when acting as the lead discipline.
Requirements:
- Active, unencumbered RN license in Vermont or an active Compact RN license.
- Graduate of an accredited nursing program.
- Minimum 1 years of RN experience.
- Hospice, oncology, or medical-surgical nursing experience is preferred.
- Valid BLS/BCLS certification.
